We put 400 mg of milk thistle in every daily serving of our Liver Detox Capsules, so we'd rather you knew exactly who it doesn't suit. This guide covers side effects, allergies and the medicines that can clash with it, with a source for each. For the bigger picture, start with our guide to supporting your liver naturally.
What are the side effects of milk thistle?
Mostly stomach complaints. The US National Center for Complementary and Integrative Health (NCCIH) lists bloating, nausea and gas as the most common side effects [1]. The Merck Manual says no serious side effects have been reported, though digestive upset can occur [2].
If your stomach reacts, take it after a meal. If the problem continues, stop and speak to your doctor.
Who should avoid milk thistle?
- People with a daisy-family allergy. Milk thistle can cause allergic reactions in people allergic to related plants such as ragweed, chrysanthemum, marigold and daisy [1][2].
- Women with hormone-sensitive conditions. Merck advises avoiding it with breast, uterine or ovarian cancer, endometriosis or uterine fibroids [2].
- Anyone pregnant or breastfeeding, unless a doctor agrees. Little is known about safety in either case [1].
- People with diabetes, unless a doctor agrees. It may strengthen the effect of blood-sugar-lowering medicines [2].
Which medicines does milk thistle interact with?
These are the interactions the Merck Manual names [2]. If you take any regular medicine, check with your doctor before you start, and don't stop a prescribed medicine on your own.
| Medicine | What can happen | What to do |
|---|---|---|
| Warfarin (blood thinner) | Its effect may increase, sometimes causing bleeding | Ask your doctor before starting |
| Diabetes medicines | Their blood-sugar-lowering effect may be stronger | Ask your doctor before starting |
| Some HIV medicines (indinavir, saquinavir) | Milk thistle may interfere with them | Ask your doctor before starting |
| Some chemotherapy drugs, calcium channel blockers and some antibiotics | Their blood levels may rise | Ask your doctor before starting |
Can you take milk thistle with alcohol?
We haven't found a listed interaction between the two, but that's a different question from whether milk thistle protects you from drinking. It hasn't been shown to. NCCIH describes trials in alcohol-related liver disease as conflicting or too small to draw conclusions [1]. A capsule doesn't cancel out a heavy weekend, which we cover in Alcohol and Your Liver.
Is it safe to take milk thistle every day?
For most adults it appears well tolerated [1]. The research is less settled on benefit than on safety: a meta-analysis of 14 placebo-controlled trials in chronic liver disease found no clear improvement in mortality, biopsy findings or most liver blood tests, while finding milk thistle safe and well tolerated [3]. So treat it as a daily wellness ingredient, not a treatment.
Does the silymarin percentage change the risk?
We haven't seen good evidence comparing side effects at 70% and 80% silymarin, so we won't claim one is gentler. What a stated percentage does give you is a known amount. That matters because NCCIH reports some supplements have held silymarin amounts far from their label, and some were contaminated with pesticides, microorganisms or mycotoxins [1].
How do we test our milk thistle capsules?
Check any brand for a named lab and a report you can read. Our Liver Detox Capsules were tested for heavy metals at Eurofins (NABL-accredited): report AR-644-2026-00000196-01, batch 25C01, dated 7 January 2026. You can see it on our Lab Reports page. That report covers heavy metals only, and it doesn't tell you whether milk thistle suits you personally.
